Home Care Coordinator

Concepts of Independence, Inc. is a non-profit consumer directed home care agency funded by Medicaid. We are the largest and oldest consumer-directed agency in New York, and one of the pioneers in the nation. We are seeking a Full Time Home Care Coordinator for our Satellite Office in Yonkers.

Responsibilities Include:

  • Consumer & Personal Assistant (PA) intake and enrollment
  • Case management: Managing and responding to consumer/PA grievances, questions, and program related needs; monitoring consumer ability to be consumer-directing; ensure that Consumers and PA’s are in compliance with CDPAP and DOH regulations and mandates; investigate and report instances of potential Medicaid fraud and other discrepancies; conduct home visits.
  • Maintain communication with Medicaid managed care plans and local Department of Social Services on any aspect relating to consumers’ services.
  • PA Time Sheet monitoring and processing.


BA/BS or AA degree in social work/psychology or related field required. Working experience in the social service/home care, not-for-profit, and/or Medicaid field a plus.

Excellent writing and communication skills required.

Basic computer skills required, with knowledge of Microsoft Word, Excel, and Outlook

Bi-lingual Spanish speaking highly preferred.

Candidate must have a valid driver’s license and be willing to conduct home visits in Westchester and Putnam Counties.
